Your First Psychiatric Visit


Please plan to arrive 15 minutes prior to the time of your first appointment to complete necessary paperwork. Please bring a list of your current medications (if any) and a list of previous mental health treatment and past medications. 

First Visit

If the patient (or parent/guardian in the case of a patient who is a minor) wishes to complete the following forms in advance of the appointment rather than just prior to the appointment, he/she may print, complete and email/fax/bring them to the appointment. Alternatively, please arrive 10-15 minutes early and we can provide these forms to you then to complete while you enjoy a light refreshment. 🙂
The intake form allows us to gather necessary demographic information and streamlines the assessment process by allowing us to focus on the most relevant issues while also ensuring no crucial information (i.e. past/current symptoms or diagnoses) are overlooked. 
The notice of privacy practices indicates the patient (and family’s) confidentiality are of course of the utmost importance.
There are also two forms allowing patient (or parent/guardian) to consent for disclosing personal health information if required for insurance or legal purposes purposes, or if the patient (or family) would like their medical record (or portions of their medical record) shared with other providers for collaboration of care.
Additional competitive prices may apply for forms, phone calls, e-mails, snail mail or in-person delivery of medication samples or gene testing kits, prescriptions, or other patient care services outside of session.
