Rafael Luces
MSN, APRN, FNP-BC, PMHNP-BC
I am a dual board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP). As a dual-certified Nurse Practitioner, I bring a comprehensive, whole-person approach to care. I recognize that mental and physical health are deeply connected and equally important.
My practice is guided by the belief that effective healthcare starts with listening. I take time to understand each individual’s story, challenges, and goals, and I tailor care plans that align with who you are, not just what you’ve been diagnosed with. I value collaboration and transparency, and I strive to make every patient feel like a partner in their own care.
In both psychiatry and primary care, I prioritize evidence-based treatment, but I never lose sight of the human experience behind the symptoms. Whether we’re managing a mood disorder, navigating a chronic condition, or working through life’s stressors, I aim to create a space that is nonjudgmental, supportive, and rooted in mutual respect.
My goal is not just to treat, but to empower. I’m here to support you in building the tools, insight, and resilience needed for long-term wellness, on your terms.
